TUITION REQUIREMENTS:

1. 1. The vehicle is fully insured for all on-road tuition and for purposes of DSA practical test.
2. 2. You must be at least 17 years old (unless you are disabled in which case you must be at least 16 and hold a valid provisional driving licence).
3. 3. It is your responsibility to ensure you have the required licence to undertake driving lessons in a motor vehicle in the U.K. You must show your licence on the first lesson.
4. 4. It is your responsibility to ensure your eyesight is sufficient to comply with driving regulations (to read a number plate with letters 3.1 inches high at a minimum distance of 67 feet or 20.5 meters. Number plates with narrower fonts introduced in 2001 should be read from a distance of 66 feet or 20 meters). On your first lesson you will be asked to read a number plate and periodically over the period of your tuition.

PAYMENTS:

1. 1. You must pay for each lesson in advance therefore on the first lesson you need to pay for 2 lessons. If you book a block of lessons payment will have to be made on the first lesson.
2. 2. You must give at least 2 working day's notice of cancellation otherwise you will be charged 1/2 the price of a lesson. Likewise if the instructor fails to give notice you will receive the next lesson for 1/2 price. This may be wavered depending on the circumstances.
3. 3. Refunds will be returned upon return of this booklet.
4. 4. The charge for the use of the vehicle on the test day will be ______

OTHER CONDITIONS

This is a list of conditions which can be agreed between The instructor (I) and The pupil(You) - delete any which do not apply - :

Punctuality: I agree to be on time for the lesson and if running late will inform You if possible. Any lost time will be made up at the end of the lesson or at an agreed later date. You agree to be on time for the lesson and if running late will inform Me if possible. Any time lost will be forfeited.

Vehicle Sympathy: You agree that having mastered the knowledge of using the car controls adopt a smooth use of those controls. In the event of causing damage to the vehicle e.g. hitting the kerb and damaging a tyre, breaking a mirror etc then a charge of _____ will be incurred.

Drugs and alcohol: If I believe that You are under the influence of either drugs or alcohol I will not conduct the lesson and you will be charged. It is your responsibility to ensure any medicines you are taking do not interfere with your ability to drive.

Fines and Points: If You incur any fines during the lesson you will be liable to pay 1/2 the amount. If You incur any penalty points during the lesson you will be liable for those points therefore it is important to keep the appointment card updated.

Tests: I will provide You with the vehicle for test provided that I am satisfied that You are test ready. If when You attend the test and the DSA cancels You will still need to pay the agreed fee (You may be able to claim some back from the DSA depending on the reason for cancellation).
